Wake Forest United Baptist Medical Center (WFUBMC)

A bag containing a document with the names and Social Security numbers of 554 patients was stolen from an employee's locked car.UPDATE (6/2/2011): The theft occurred on February 15 at an outpatient clinic parking lot.
